This is a recorded session from the SGIM 2024 Annual Meeting in Boston.
While many regional health improvement collaboratives are focused on quality improvement (QI) in primary care, few statewide initiatives exist. State-level initiatives avoid the cross-state policy barriers associated with larger multi-state initiatives and have the potential for greater impact than regional efforts. The Agency for Healthcare Research and Quality (AHRQ) funded four states with high cardiovascular events to build sustainable statewide primary care collaboratives implementing patient-centered outcomes research (PCOR) findings to improve cardiovascular care delivery and outcomes. This panel will share lessons learned from each state for building strong relationships with diverse stakeholders, establishing collaborative QI infrastructure, engaging practices, and statewide dissemination and implementation of PCOR to improve cardiovascular health and cardiovascular health equity.
- Identify ways to strengthen relationships with diverse stakeholders
- Determine strategies to form statewide primary care collaboratives representative of the communities served to improve health equity
- Discuss use of codesign approaches to develop relationships and quality improvement initiatives
- Identify successful practice facilitation approaches to support quality improvement initiatives
