Addressing Physician Conflict of Interest in Medical Education and Research

This interest group is for physicians who are concerned about the influence of the pharmaceutical industry in medicine and the potential conflict of interest these relationships create. Industry influence can be bad for public health because it can raise the costs to patients by promoting inappropriate prescribing of expensive drugs. Pharmaceutical representatives—who often have personal relationships with physicians and their office staff—often persuade physicians to prescribe newer and more expensive medications when cheaper and equally good alternatives exist. Join us for a discussion on these issues including creating an pharm-free practice; examining institutional academic–industry relationships and calling for transparency in payments for speakers, research, and lunches; promoting industry-free continuing medical education (CME); supporting a legislative ban on the sale of physician prescribing data for marketing purposes; and calling for registries to create transparency in industry gifts to individual physicians.

Anticoagulation/Thromboembolism

A number of SGIM members focus much of their clinical efforts on anticoagulation clinic care and/or the evaluation and treatment of patients with thromboembolic disorders. A sizeable number of members are actively engaged in clinical research in the fields of anticoagulant therapy and thromboembolism. The Anticoagulation/Thromboembolism interest group is an informal gathering of SGIM members who have an interest in this area of clinical medicine. Principal goals are a) discussing potential collaborative projects, b) review of common issues regarding the delivery of high quality anticoagulation care, c) update on new topics in the field of thromboembolism, and d) presenting results of research studies of interest to the group.

Clinician Examination--Research and Education

The Clinical Examination Research Interest Group focuses on the Rational Clinical Examination Series (published in JAMA), knowledge dissemination through monthly updates on a listserv and the SGIM webpage (http://www.sgim.org/clinexam.cfm), and clinical research through the CARES group (http://www.carestudy.com/). We come together at the SGIM Annual Meeting to work to a) coordinate activities that include involvement in the Rational Clinical Examination Series of the Journal of the American Medical Association, b) plan yearly activities, and c) provide a means for social networking among those interested in the clinical examination. The International Cooperative Group for Clinical Examination Research investigates, documents, and disseminates scientific evidence that supports or refutes the accuracy and precision of the clinical interview, physical examination, and bedside tests toward the provision of efficient and efficacious health care.

Complementary and Alternative Medicine

The purpose of this interest group is create a forum for members of SGIM who have a desire to discuss research in CAM, graduate and post-graduate CAM curriculum development, and professional development involving CAM modalities. Through this forum we wish to develop resources that will facilitate collaboration among SGIM members in both research in CAM and in educational development. Our ultimate goal will be to help members identify: 1) other members of SGIM who have an interest in CAM, 2) funding sources for research/education 3) educational resources for graduate, post graduate, and professional education, and lastly to act as a point of contact between SGIM members interested in CAM and members of other medical organization who have a similar interest in CAM.

Cross-Cultural Communication and Medical Care

The Cross-Cultural Communication and Medical Care Interest Group has been formed to serve as a clearinghouse, a space for inter-institutional collaboration, and a forum to promote collegiality for those doing work in a variety of areas. These include (but are by no means limited to) curriculum development, research, and evaluation in areas of cultural aspects of communication, working with interpreters, and related areas of the medical encounter. We invite all interested SGIM members to join our work and contribute to the improvement of educational interventions, services, and standards in Cross-Cultural/Culturally sensitive care.

End of Life

We welcome anyone with an interest in hospice, palliative, or end-of-life care. A broad range of experience levels will likely be present. Time will be available for networking, career mentoring, and planning future collaborative projects. Over the past 3 years, 9 clinical updates or workshops have been presented by interest group members on topics like spirituality, update in palliative medicine, conflict resolution, symptom management, and teaching palliative care.

Ethics

This interest group provides a forum to discuss and exchange ideas about medical ethics and humanities. We work to promote education about these topics in the medical school curriculum, to promote related research by SGIM members, and to improve the visibility of scholarly work in ethics and humanities within SGIM. The interest group time will provide the opportunity to network with other SGIM members who have an interest in these areas. We will discuss ethical issues facing SGIM members in their teaching and medical practice and those facing SGIM as an organization

Evidence Based Practice

Evidence Based Medicine is currently a mandated curriculum topic for internal medicine residencies, and a natural area of education interest and activity for many SGIM members. We have encountered a number of challenges inherent in teaching our trainees the practice of EBM, and will lead the development of an interest group that will provide a forum for elucidation of these problems, and identify potential solutions and areas for future research.

Gay and Lesbian Health

This interest group, formed at the May 2000 Annual Meeting, provides a forum for physicians interested in gay and lesbian health care issues. The group shares an interest in curriculum development, research, and clinical care as it pertains to the GLBT community. We welcome anyone interested in developing innovative methods that enhance the teaching of gay and lesbian health issues or who simply wants to learn more about providing comprehensive and compassionate care to his or her gay and lesbian patients. We will continue to discuss innovative curricular ideas for teaching medical students and residents about issues relating to gay and lesbian health care needs. In addition, this group will provide a forum for physicians involved in these efforts to share their experiences.

Genetics in Medicine

Homeless Healthcare

The Homeless Healthcare interest group was founded in 2001 to provide a forum for SGIM members interested and involved in clinical, educational, policy and research activities with homeless patients. Our meetings provide an opportunity for members to share ideas about improving care, creating and evaluating curricula, engaging in advocacy and supporting research efforts. At this year’s meeting, we look forward to discussing potential collaborations in clinical guidelines, educational initiatives, policy advocacy and research projects. Home Visit

Hypertension

The SGIM Hypertension interest group provides a forum for collaborative exchange for those members of SGIM interested in the care of patients with hypertension, research in hypertension, educational opportunities in hypertension, and health care policy in regards to hypertension. During this year’s interest group meeting, we will discuss the results of our recent survey of Chiefs of General Internal Medicine regarding their attitudes towards and uses of hypertension clinics and the availability of ambulatory monitors for HTN care. Medical Consultation

Perioperative medicine is an important aspect of medical consultation. We will provide a brief status report of the working groups formed at previous meetings. Specifically these groups deal with writing projects, collaborative research, and curriculum development. (The perioperative medicine literature review is now a separate Update Session). There will be a presentation of how different consult services are designed, and the group can discuss how their services run and get new ideas to incorporate. Interesting or difficult patient management cases will be discussed as well as the new guidelines for perioperative beta-blockers. Audience participation is encouraged. The use of our listserv and website will be discussed, and new attendees can be added to it.

Medical Informatics

The medical informatics interest group will discuss several activities of interest, including educational events on clinical informatics topics, development of a resource on the web for informatics related tools and promoting awareness of informatics as a discipline and how to engage stakeholders ranging from providers to patients to policy-makers. Some topics of importance expressed by our members include informatics concepts (databases, standards, workflow issues, nomenclature, privacy and security etc), applications (electronic health records, clinical decision support, health information exchange) and clinical informatics education (AMIA 10x10 program, informatics fellowships etc.).

Mental Health in Medicine

General internists are increasingly responsible for managing mental health issues in their patients. The Mental Health in Medicine Interest Group, now in its 7th year, provides a forum in which general internists can meet to exchange knowledge about the interface of general medicine and mental health care. The interest group aims to facilitate collaborations among researchers and educators in this field. Furthermore, the group is interested in advocating for higher quality clinical care for general medicine patients with mental health problems. The group welcomes all junior and senior researchers, educators, and clinicians with an interest in mental health issues in both the primary care and inpatient settings.

Obesity Prevention and Treatment in GIM

Obesity is one of our nation’s most important public health challenges. There is a growing interest and need to better understand and manage obesity in academic medicine. Many generalists are working on clinical and research projects to address this issue. Through this forum, we will create a setting for participants to network with colleagues who share an interest in obesity prevention and treatment; report existing projects and/or proposals; exchange ideas and resources for program development and evaluation; explore creative approaches to increase the presence of obesity-themed work at SGIM; potentially collaborate on future projects.

Obstetric Medicine

Rising maternal age and obesity have increased that number of women with medical disorders during pregnancy to 35%. In addition to internists being called to diagnose and treat this population, there is a need to help prepare women for pregnancy when they have established medical issues. In contrast to preconception “counseling” that addresses potential risk issues, preparing women for pregnancy with “Preconception Planning” by internists is a concept that has come of age. Additionally, we need to understand how to address the long term health implications of pregnancy complications such as gestational diabetes and preeclampsia.

This interest group provides a forum within which SGIM members may meet clinicians, educators, and researchers who also share an interest in this field of Women’s Health. The 2010 meeting will include a review by Dr. Tanya Melnik regarding acute leukemia during pregnancy. Other topics will include discussion of a quality improvement project regarding use of and counseling about ACEIs and prenatal vitamins to women of childbearing age in a general medical clinic, ways to improve Preconception Planning, interconception, and postconception care among SGIM members, and the specific methods SGIM members can use to build an Obstetric Medicine service at their institution.

Pain Medicine

To assemble a group of General Interests who have clinical, educational and research interests in the field of Pain Medicine. The group will meet annually at the SGIM national meeting to discuss new developments in pain medicine. The goal of the meeting will be to organize and develop precourses and workshops for future annual meetings. Other activities will be to develop clinical guidelines for treatment of pain in primary care settings, sponsor educational activities at national and regional meetings and develop collaborative efforts on research projects and manuscript development.

Spiritual Care Guidelines

In 2009, an innovative model for the implementation of interprofessional spiritual care was developed by a national consensus conference. The resultant model and recommendations for the integration of spiritual care in clinical settings was published in Journal of Palliative Medicine October 2009 and also as a book entitled Making Healthcare Whole: Integrating Spirituality into the Care of Patients.This report has been receiving widespread attention; currently there is a national initiative were hospitals are integrating spiritual care projects in their setting. The interest group will look at ways primary care can integrate these guidelines in their settings as well as develop resources specific to primary care, including educational resources.

Substance Abuse

The group is made up of general internists with educational, clinical or research interest in unhealthy (the spectrum from risky use through dependence) alcohol, tobacco and/or other drug use, who collaborate to advance academic endeavors. Over the past 2 decades, this growing group has sponsored educational presentations at national and regional meetings, stimulated research collaborations, fostered mentoring relationships, encouraged joint educational projects including peer reviewed publications, represented the Society on policy issues, and developed warm friendships between colleagues.

The group is interested in attracting new members and has traditionally been a useful vehicle for junior faculty to develop connections with established faculty in the field. The group is a potent force improving the quality of care and moving the care of the substance abusing patient into mainstream internal medicine. The group also meets annually in the fall at the Association for Medical Education and Research in Substance Abuse (AMERSA) national meeting.

Transfers and Handoffs

The transfer of care among providers is a complex process that is increasing in frequency and is a frequent cause of medical error. Furthermore, the Joint Commission's new mandate that hospitals ensure "safe and effective hand-offs" is difficult both to implement and to measure. We hope this new interest group will provide a networking opportunity for educators, researchers and administrators interested in improving hand-offs. Discussion may include solutions to common problems, curricular innovations, scheduling changes, assessment instruments, standardized formats and tools, and ideas for research and collaboration.

Transition of Care

This interest group is for those individuals interested in issues related to the transition of adolescents and young adults with chronic disease from the pediatric health care system to the adult health care system. The group’s goals are to advocate for increased attention to this area of health disparity through education of general internists to the unique needs of these patients and through research into best practices for providing quality care.

VA Primary Care

SGIM includes a large and diverse group of VA clinicians, teachers and researchers interested in primary care and general internal medicine. The interest group provides a forum for VA-affiliated faculty members to share their VA perspective on interactions with an academic medical center. This allows them to share approaches to their dual role as VA and University affiliates that have allowed them to maintain a balance. The many VA primary care clinician educators who belong to SGIM also benefit from the opportunity to share approaches to the common issues of improving quality and access.
The VA Primary Care Research Interest Group provides a venue for information exchange regarding the shared research interests of VA Primary Care researchers. This peer group is especially important for SGIM members who work in VA facilities away from the traditional centers of VA research.

Women's Health Education

There has been a growing interest among educators in Internal Medicine in the area of Women's Health and gender-based medicine. Multiple groups, including the Consortium For Women's Health Medical Education, the National Academy of Women's Health Medical Education, the SGIM Women's Caucus, as well as individual faculty members, have worked on this issue, but until now there has not been a national focused working group of internists committed to developing resources and educational programs to expand internists' skills.

The Women's Health Education Interest Group was started five years ago to enable individuals involved in women's health education to network and interact with each other. The mission of the Women's Health Education interest group is to provide a forum for faculty to network, to become informed about current women's health education initiatives, to foster multi-institutional collaborative efforts and to identify strategies for continued integration of women's health and gender based education into undergraduate, resident and faculty development curricula.

Criminal Justice and Health

The Criminal Justice and Health Interest Group is designed for clinicians, educators, researchers, and policy makers interested in the criminal justice system and health. We seek to foster discussion and collaboration related to patient care, research, education, advocacy related to health and the criminal justice system. In the area of patient care, topics of concern include: addressing the burden of chronic disease, infectious disease, substance dependence, and mental illness in criminal justice populations; fostering collaborations among correctional physicians and community physicians who care for patients with a history of incarceration; optimizing the transitions of care between the community and correctional health systems; and understanding the legal, economic, and cultural challenges to achieving a community standard of care and adhering to clinical guidelines.

With regard to areas for further research, we aim to foster a greater understanding of the potential role incarceration may play in disparate health outcomes for populations impacted by the criminal justice system. Further, we aim to address the legal and ethical challenges of conducting research in this protected population. We aim to foster collaborations with government and non-profit agencies that serve this population to improve the available evidence base. The interest group is also designed as a forum to share educational opportunities for medical student and resident trainees and to incorporate correctional health topics into existing curricula. Further, we will serve as a resource to learn about advocacy and policies that potentially have an impact on health of correctional populations and their communities. The ethical and professional dilemmas faced by physicians practicing in correctional settings are also of interest.
